As for the actual appointment, things won’t feel rushed, and our team will take their time thoroughly examining and tending to your smile, and you’ll have plenty of opportunities to ask questions and receive genuine answers. Our goal is to make sure you leave your first visit feeling heard, comfortable, and confident that you've found a practice you can call home!
Patient Forms
We know that your time is valuable, so we’ve made it as easy as possible to complete your new patient paperwork before you even arrive. All of our patient forms are available online, allowing you to fill them out at your own pace from the comfort of home. Simply complete and submit your forms prior to your appointment, and our team will have everything ready when you get here.
